Thank you for your interest in becoming a participating provider in AmeriHealth’s provider networks.

All providers interested in participating in our networks must complete and return a credentialing application, as explained in Network participation and credentialing.

Information obtained during the credentialing process is confidential and will not be shared with third parties. Submission of the credentialing application and required supporting documentation does not guarantee inclusion in the AmeriHealth network(s).
