The How Have Your Wellness Habits Improved Your Life? essay contest is sponsored by AmeriHealth Insurance Company of New Jersey and AmeriHealth HMO, Inc. (together, “Sponsor”), 8000 Midlantic Drive, Suite 300N, Mt. Laurel, NJ 08054.
The essay contest is open only to legal residents of The United States of America, 18 years of age and older as of the date of entry. Employees of Sponsor, their dependents and members of household are eligible to win.
The essay contest is open until Friday, October 22, 2010 at 8:59 a.m. (ET). To enter, send all essays, via email to jodie.kirsch@amerihealth.com. Limit one entry per person. Multiple entries will be disqualified. You must supply a valid e-mail address to win. If the e-mail address supplied is not valid, your entry will be disqualified.
One (1) first prize winner and one (1) second prize winner will be selected from all eligible entries. To qualify, entries must be received no later than 8:59 a.m. (ET) on Friday, October 22, 2010. The winners will be announced on or about Friday, November 5, 2010. Winners will be notified via e-mail and/or telephone. Winners must respond to e-mail or voice-mail notification within 24 hours, or he/she will forfeit the prize. If a prize is not claimed within the time limit, an alternate winner will be selected from all remaining eligible entries. Prizes are not transferable and cannot be redeemed for cash, and no substitutions are allowed. Odds of winning the prizes depend on the number of eligible entries received. If (i) any prize or prize notification is returned as undeliverable, (ii) winner declines his or her prize, or (iii) winner is unable to attend the event on the date specified, it will result in the disqualification of such winner and an alternate winner may be selected by Sponsor at its discretion. Sponsor will use reasonable efforts to pick an alternate winner but, in the event an alternate winner cannot be determined, the prizes will go unawarded. Sponsor is not responsible for late, lost or misdirected entries or for telecommunications system, computer-software or hardware errors or data loss.
One (1) Prize: Winner will receive a $200 Visa gift card. The second place winner will receive a $75 Visa gift card.
Online entries by Internet will be declared to have been made by the authorized account holder of the e-mail address submitted at time of entry. “Authorized account holder” is defined as the natural person who is assigned to an e-mail address by an Internet access provider, online service provider or other organization (e.g., business, education, institution, etc.) responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. Sponsor is not responsible for entries not received due to lost, failed, delayed or interrupted connections, including Internet/Web-server/network/phone-line connections or miscommunications, or other electronic malfunctions, computer or telephone hardware or software failures, malfunctions, or garbled or jumbled transmissions. Sponsor reserves the right, in its sole discretion, to cancel or suspend this essay contest should virus, bugs, unauthorized human intervention, or other causes beyond the control of Sponsor, in Sponsor’s sole opinion, corrupt the administration, security, fairness, or proper operation or play of the essay contest, in which case winners will be selected from Internet entries received prior to action taken. All entries become the property of Sponsor and will not be returned to the entrants.
Void where prohibited by law. Subject to all federal, state, and local laws. By entering, participants agree to be bound by these Official Rules and the decisions of the Sponsor on all matters relating to the essay contest, which decisions are final and binding. Entrants agrees to release Sponsor and its affiliates and advertising/promotional agencies from any and all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to person or property that may be sustained in connection with the receipt, ownership, or use of a prize or with participation in this essay contest. If for any reason the essay contest is not capable of being conducted as described in these Official Rules, Sponsor shall have the right, in its sole discretion, to cancel, terminate, modify, or suspend the essay contest.
By entering this essay contest, the entrant agrees that the Sponsor, its affiliates, advertising and promotional agencies, and the employees, officers, directors, and agents of any of the above organizations shall not be liable for any injury or damage to any person's computer hardware, software, or peripherals utilized in participating in the essay contest. Sponsor and its affiliates are not responsible for technical, hardware, software, or telephone malfunctions of any kind, for lost or unavailable network connections, or for any programming-associated failures or malfunctions.
By accepting a prize, winners automatically grant to Sponsor the right to use their proper names, images, and likenesses without further permission or financial compensation for any promotional purposes unless prohibited by law.
Sponsor reserves the right to disqualify any person it finds to be acting in violation of these Official Rules or to be acting in an un-sportsmanlike or disruptive manner, or with intent to annoy, abuse, threaten, or harass any other person. CAUTION: ANY ATTEMPT BY A CONTESTANT OR ANY OTHER INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE ESSAY CONTEST IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, THE SPONSOR AND ITS AGENTS RESERVE THE RIGHT TO PROSECUTE SUCH PERSON AND/OR SEEK DAMAGES FROM ANY SUCH PERSON TO THE FULL EXTENT PERMITTED BY LAW.
